my 6 month "diet" required by insurance

I know that each individual insurance may require you to do a exercise/nutrition diet for a period of time....I was wondering if others had to do this and after you completed it were you typically denied/approved. I am worried about being denied, but if i am will do a appeal letter if need be....thanks guysAisha :)
