There is a wonderful place off of 52 in Wilmington, DE that does all that. The short name is PMRI, I think it stands for Pain Managment Rehablitation Institue they are a part of Christian Care Helth Care systems. They have a gym on site and specialize in both Cardiac and Gastric By-pass rehablitation. I don't have the number right now but they are in the Wilmington, DE phone book.
