One suggestion I have to help you get past the carb addiction that has no doubt set back in is do the 5 day pouch test - you do 2 days of protein shakes, a day of mushies, a day of soft proteins and a day of solids. As of a month ago, I had gained back 15 lbs from my rock-bottom (132), and 10 lbs from my comfort range. So I went on the Whole 30, essentially a fairly restrictive 30-day Paleo food plan, really intended to be an elimination diet to help people figure out what foods they react to. There's a blog, Whole 9, that has the Whole 30 program, or you can buy a book, It Starts With Food.
Within 3 weeks I had lost 12 lbs, and by the end of the 30 days I had lost 14. And feeling really great, energetic and much less arthritis pain. I'm now past the 30 days but still eating pretty much the same as I was on the program. I will likely add back in some bacon (which is off plan for the 30 days because it contains sugar), and possibly a bit of cheese as a condiment, like Parmesan or feta. (But no longer the meal-based cheese I ate before, mostly Gr yogurt and cottage cheese for breakfast).
Even if you don't do a Paleo-style plan, going back to basics will help. Journal what you eat (and be ruthlessly honest), stick to mostly proteins, veggies, healthy fats and a little fruit, establish a calorie limit. You can do this!
