APPROVED!!!!
Today is such a great day!!! After waiting and waiting, and test after test, and a denial and then an appeal, I've FINALLY been approved! I'm so very thankful! Next step is to schedule my pre-op appointment with my surgeon and then schedule my surgery! Thank you so much for the good thoughts and prayers. 
I started this whole process in August. My doctor referred me to the surgeon and I had my first appt with him in September. I had to complete a 6 month diet which I finished in February. Also I had to complete a cardiac and pulmonary clearance along with blood tests and an endoscopy. We submitted everything to insurance and they denied it saying that I didn't have proper pulmonary clearance because my family doctor signed it, not a pulmonary doctor. I then had to see a pulmonary doctor and have a sleep study done along with a pulmonary function test. Everything came back good so we appealed the insurance decision. They denied again stating they wanted to see sleep study results. We appealed again and resubmitted all paperwork including a copy of all test results on 4-21 and FINALLY was approved today! I don't know that the process is always as lengthy as mine was though so fingers crossed you get good news MUCH sooner than I did
