Stupid profile question

How do you get the info on the upper right side on your profile (date, surgery, surgeon, etc. ) It's probably obvious somewhere but I just don't see it when I try to edit my pofile.

I am 4 1/2 weeks from my surgery date and so ready for this. I ran an errand at lunch today and stopped for takeout chinese since I thought it will be a long time if ever, before I eat chicken with cashews again. Now I just feel sick after eating that. You know what? I can live without this stuff. Bring it on!
