I have Aetna EPO and I was approved for a revision and Im scheduled for surgery on 4/21. The process of approval was the same for the revision as it was for my original wls.

I have Magnacare... anything prior to them is like a slate wiped clean. they said and I quote that first surgery had nothing to do with us.... this is a new policy means new coverage,,,, so I got approved very quickly.Good Luck

 I had UHC PPO.  However, its really going to depend on what your employer signs up for.  What you will likely need to do is contact each of the insurance companies at open enrollment time and ask if WLS revisions are covered.

Disclaimer: I am not an insurance guru.  (We do have a few of those.) 

Sometimes the insurance company can be made to cover IF your present WLS is causing possible life-threatening conditions OR has a mechanical failure.  Look through your complete policy for that and also get copies of all policies available before open enrollment.
