Unfortunately, I had to delete this post due to privacy concerns.

Got a lap band in 2008. Tried hard, but didn't lose much weight & developed swallowing problems. Fought my insurance company for almost a year & finally had a band to DS revision on 5/11/12. Have now lost 125 pounds. Yay!

The average price my company bills the insurance is approx $500.  This, however, is not what the insurance allows.  When you call the insurance, ask them what the "allowable amount" is for this test and make sure if it needs pre-authorization that the doctor's office or wherever you are getting it done gets this.  Also check to see if you need a referral from your pcp.

The amounts charged to the insurance company by a specialist is based on the CMS Medicare allowable amount and multiplied by 3.  A pcp multiplies by 2.  Then it all depends on the practices contract with the insurance company as to the allowable amount.  Also depends on your employers contract with the insurance company as to how it's covered and how much is covered.
