I believe you will have to go to Texas. You need to get your EOB and see how to do it. You will have to fight for it.

It is very difficult to find DS surgeons anywhere who accept Medicaid. The reimbursement rates are so low that it's hard to find doctors in any field who accept Medicaid, as you probably know. How far are you willing to travel?
