Exactly what documentation

This may sound a little ignorant, but exactly what kind of documentation from PCP is needed to send in to the insurance company? Is a letter from the doc. stating that I was on a certain diet and what my weight was for three months sufficient? OR do insurance companies want copies of exact medical records? Please help!! Insurance is Humana HMO.
