Does anyone have United Healthcare PPO......

Can you tell me what type of paperwork is needed so that I can move my process much faster....

I have UHC-PPO and although my doctor submitted all of the paperwork, I know that they sent in a letter detailing my need, medical history, 5 year diet history and "mini-psyche" evaluation. Although they do not require a referring physician for this type of policy, I had my family doctor refer me to the surgeon and we sent in his letter too. The result: I was denied. I have sent in an appeal. They said it was an exclusion listed in my policy, but the policy exclusion really says "weight control or treatment UNLESS there is a diagnosis of morbid obesity." Be prepared! I wasn't at all prepared for this denial. I meet all of the criteria. Hopefully, someone with a heart will understand how ill I am from my obesity and push this through for me.
