Also B1. I've been noticing a similar thing, and I went to the doctor and that's what they told me. I take an extra supplement and I seem to notice a big difference.
I'm also going to start taking weekly b12 shots. Hopefully with the 2, I can start retaining things better.

Get copies of your labs.  I'd be looking at my B12, B1 and B6 levels immediately as the memory loss is one of the signs of B vitamin deficiencies.  Do NOT take your docs word for everything being ok.......just because it's in the range of the lab does not mean it's enough.  Please be diligent in getting this done; permanent damage is a possibility if you are deficient.

I also recommend you get the D3 dry form, RX D is d2 (oil based) and not a good option for us.
