03/23/04 ~ I'm a 39-year-old married woman with a 4-year-old daughter. Was never "skinny" as a child; however, I put on most of my weight right after graduating high school. I remember being around 160-170 right when I graduated. Definitely not my ideal weight. In March of 2004 I began researching gastric bypass surgery. Met with Dr. Kaczmarski of The Bariatric Treatment Center in Langhorne, PA today. I was extremely impressed with The Center, and found Dr. Kaczmarski to be very personable. He took extra care to explain everything in great detail. While I had been hoping to have the procedure done laparoscopically, he explained that because I had a prior splenectomy and there would be residual scar tissue, it would be safer to do an open RNY. I felt very comfortable with everyone at The Center, and I would highly recommend them to anyone who is thinking about visiting. I have to get the necessary paperwork for Aetna now, which includes an Echocardiogram (due to the fact that I took Fen/Phen for 18 months back in '95/'96) and clearance from my hematologist (since I have ITP -- low blood platelets). No psychological evaluation is required, and no sleep apnea test is required. I think I am going to run into a problem meeting Aetna's requirements, as I don't have at least six months of a physician-supervised diet within the last two years. Everything I've tried has been "on my own". I hate to think I will have to start YET ANOTHER diet and wait around six months before applying to Aetna for the approval. I am feeling pretty down about this. Any advice anyone might have would be greatly appreciated.

 

04/02/04 ~ Got the letter of medical clearance from my hematologist today! One step closer...

04/08/04 ~ All test results came back and everything is perfectly fine!


04/05/04 ~ Had my Echocardiogram and Stress Test today. The Echocardiogram was a breeze. The Stress Test was another story...they got the first needle in, but my vein kept moving around, so they had to redo it. Let me tell you - it was the WORST needle stick I've ever had. After the injection, you wait 20 minutes for the radioactive stuff to circulate. Then they lie you flat on your back while a camera circulates around your chest, taking pictures of your heart. This takes about 20 minutes. Well, thanks to my lovely anatomy, the camera was not able to get clear pictures. So they had to turn me over and redo the pictures (another 20 minutes). Then I went on the treadmill until my heart rate was at 92%. They only needed it to get to 86%, but mine got up there quickly. That part was a breeze. Then you cool down for 15 minutes, and they redo the pictures. I had to have them redone both on my back and on my tummy, so the cardiologist can compare both sets. Four hours later, I finally walked out of the hospital. Once I get the results in a few days, I will have all my paperwork complete and it will in the mail to The Bariatric Treatment Center. Then everything will go to Aetna for approval.

 


04/26/04 ~ Received my denial letter from Aetna today. I was denied due to lack of documentation of a six-month physician-supervised diet (same reason most people are denied by Aetna). I knew this was coming, though. It's okay though ~ I did not gain all this weight overnight, so I don't expect to lose it all overnight either. What's another few months? I can wait. I am now starting Aetna's 3-month multi-disciplinary pre-surgical regimen, which consists of nutritionist visits, a reduced-calorie diet program supervised by nutritionist, behavior modification, and an exercise regimen.

 

05/04/04 ~ I joined Curves for Women today. I spoke with Aetna member services who advised that going to Curves would cover the exercise regimen as long as my PCP and nutritionist make note of it in their files.

 


05/13/04 ~ First appointment for "behavior modification". Wonderful counselor; very understanding. I think she is going to be a huge help to me. I am excited about having someone "neutral" to discuss WLS with. She is definitely not anti-WLS. I will see her once a week for the next three months to be sure I cover all of Aetna's requirements.

 


05/20/04 ~ First appointment with the nutritionist is set for Monday, 5/24. Immediately afterwards, I have an appointment with my PCP to be sure everything is clearly documented.

 


06/04/04 ~ Have been going to Curves for a month now and check out these results:

Weight Lost -8.00 lbs.
Inches Lost -15.50 inches
Body Fat Lbs. -2.16 lbs.

Bust -3.50 inches
Waist -3.00 inches
Abdomen -5.00 inches
Hips -0.50 inches

Calves 0.00 inches (not sure why I lost nothing here!)

Still going for behavior modification every week, and have my next appointment with the nutritionist in a month. Am about DISGUSTED with Barix Clinics (formerly Bariatric Treatment Centers). They claimed they never received my denial letter, so I faxed it over to them. Have left three messages there, and to this day NO ONE has called me back. I finally called the rep. from Michigan (the one who sent her business card when the initial brochure came in the mail) and of course she said someone would call me but they haven't. I'm pretty disappointed in their lack of follow-up. They have no idea I'm doing this three-month pre-surgical regimen. Unbelievable. Have thought about going to meet other surgeons but they all want a consult fee upfront, and I can't afford to keep paying all those fees (who could?).

 


08/04/04 ~ I've completed the three-month multidisciplinary pre-surgical regimen, as required by Aetna! I will be seeing my PCP tomorrow and Barix Clinics will be resubmitting everything to Aetna. Please say a prayer for me ~ I really feel this is my last hope. On a side note, I have lost a total of 20.25", 8.75 lbs., and 3.73 body fat lbs. in three months at Curves. While I am pleased with those results, it would take me YEARS to get to the weight I want to be at. My BMI is now 40.0 (still morbidly obese).

 


08/11/04 ~ All documentation has been sent from my PCP to Barix Clinics. Spoke with Joyce @ Barix today, and she assured me she would fax everything right to their Michigan office just as soon as it's received. Wish me luck!

 


08/30/04 ~ I decided to call Aetna today and see if they had any status for me. Get this - they have received NOTHING from Barix. I then telephoned Barix in Langhorne, PA and was told to call their insurance department in Michigan. When I spoke to them, they told me they had not received ANYTHING from the Langhorne office! UN-F*CKING REAL! The super-sweet woman I spoke to assured me that Rena (the insurance gal in Michigan) would call me right away to find out what has happened to my paperwork. I AM SO TICKED --- here I was thinking I would have an approval or denial any day now, and the dang paperwork hasn't even been sent. I must say though, I've read of several others having this same problem with Barix. I am thorougly disgusted. I telephoned my PCP's office who confirmed that the packet of information was mailed to Barix on 8/6 using the envelope THEY provided me! I then called Barix in Langhorne again and left a message for Joyce Adamski, Patient Counselor. Joyce returned my call immediately and informed me I was the third person this week whose paperwork was sent from their PCP but not received at Barix. Okay, so where the he!! is the paperwork going then! She advised me to have the PCP's office send everything again, even by fax. I called my PCP's office back and they are getting everything ready again to be faxed first thing in the A.M. I am SO frustrated right now - I am truly feeling that this just wasn't meant to be for me

 


08/30/04 ~ Rena from Barix in Michigan called to confirm they received everything (via fax) from my PCP. She asked if I wanted everything sent to Aetna now. Well, duh, of course I do!

 


09/09/04 ~ Decided to call Aetna today for a status. Well, the first rep I spoke with could not find ANYTHING in their system for me (the most recent documentation they had was my original denial back in April). She advised me to call the surgeon's office and be sure they sent it. HERE WE GO AGAIN... I then telephoned the Michigan insurance office at Barix who confirmed they sent everything via Certified Mail on 8/31. Okay, I then called Aetna back and got a very sweet gentleman on the phone who looked through all their logs, including those from the Appeals Office, and he was unable to locate anything. He asked if the information had possibly been sent directly to Aetna's Patient Management office in Blue Bell, PA. Heck if I knew ... He then suggested I call them and see if they had anything there. Okay, fourth phone call. I spoke with "Stan" in Patient Management/Pre-Cert who initially thought I was a doctor's office calling. Once he realized I was the patient, he said he could not provide me with ANY information. I was told the surgeon's office had to call Pre-Cert for a status. OKAY! WTF? Why did no one at Barix tell me this? Fifth phone call ... to Rena at Barix in Michigan. Explained everything to her, and lo and behold, she tells me "Yep, I've got it marked on my calendar for 9/21 to follow-up with Aetna." Again, I wish I knew that was the way this thing worked. Rena assured me (dare I use that word?) that as soon as she heard something from Aetna on 9/21 she would call me. I'm not holding my breath ...

 

09/21/04 ~ Called the Insurance Office at Barix in Michigan. Was told that Rena was out of the office on 9/20 so she had not yet called Aetna on my behalf, although it was in her "to do" pile. I requested that she call me as soon as she heard something, one way or the other.

 


09/22/04 ~ No return phone call from Rena at Barix yet. I called the office again this afternoon and the individual I spoke with saw "no notes" on my file. She then transferred me to Rena's voice mail. Within ½ hour, I got a call from Rena. She went round and round with Aetna, and was basically told it's still under review. She then asked me if we were going to submit under my secondary insurance, Horizon BC/BS of NJ. I told her that I thought we couldn't do that unless my primary absolutely did not cover the surgery. She was not sure but said she is going to go ahead and submit everything to them tomorrow. Maybe I'll get lucky and they'll approve me!

 


10/05/04 ~ I was still waiting on Aetna (my primary insurance) to make a decision on my Appeal. They have been dragging their feet... On 10/1, Barix submitted to my secondary (Horizon BC/BS of NJ). Not sure why they didn't submit to them back in April but anyway...I called Horizon today for a status - I am approved 100% !! They are in-network!! They already faxed an approval to Barix! The lady in the insurance department at Barix did say we MIGHT need to wait for Aetna's decision anyway, but she would have Rena contact me tomorrow for sure. I probably won't get a surgery date until January, but that is fine by me! I absolutely cannot wait to join everyone on the losing side!! Thank you, one and all, for the support, advice, suggestions, etc.!!

 


10/16/04 ~ Well, after my initial denial in April, I'm finally approved! Funny part is, I found out two weeks ago my secondary insurance agreed to cover the surgery 100% in-network anyhow, but I still had to wait for Aetna's decision to simplify things. Today, I received a letter stating: Dear Ms. XXXXXXXXXXX: Aetna Life Insurance Company (Aetna) is in receipt of a letter filed on your behalf by XXXXXXX of XXXXXXXXXXX requesting coverage for gastric bypass surgery (procedure code 43847). All of the submitted documentation pertaining to the above-mentioned request was reviewed by a physician who is board certified in general surgery. It was the determination of this physician to overturn the previous decision and to authorize coverage for gastric bypass surgery, procedure code 43847. This determination is based on the fact that with the new information submitted it is shown that your Body Mass Index is 40.5, you are 38 years old, you have attempted weight loss in the past without successful long term weight reduction, and you have recently completed the 3 month multi-disciplinary surgical preparatory program requirement as noted in Aetna Clinical Policy Bulletin #157 (Obesity Surgery), which was referenced for this determination. As such, you have met the criteria for gastric bypass surgery. WOO HOO!!! I will definitely be waiting to schedule until after the holidays though (my personal choice).

 


10/18/04 ~ I just wanted to let you know that I spoke with Aetna Member Services this morning who confirmed for me that my authorization is good for six months from the date of approval. They also confirmed that although the surgery may not be covered for SOME after 1/1/05, I am "grandfathered" in with my approval letter. I then spoke with my surgeon's office who also verified this with Aetna. They also told me that even if I couldn't have my surgery within the six month timeframe, they would simply call Aetna and request an extension, which is always granted. The only time we would have to refile is if I waited over a year. Just an FYI for anyone dealing with Aetna. Do not panic!

 


10/20/04 ~ Received a call from Amy at Barix who wanted to confirm my clearances that Dr. Kaczmarksi requested. I informed her that I'd already provided the clearances necessary back in April.

 


10/21/04 ~ Received a call from Amy at Barix. She confirmed my approval and advised that I am in "benefits review", meaning they are now determining what is/is not covered through both insurances, so I will know upfront what is/is not covered. She advised I should receive a call from Audrey within the next two weeks to schedule my date. About ½ hour later, Amy called to ask me if I'd quit smoking. I almost fell off the chair! I've never smoked in my entire life! Somehow it was written in my chart that I smoke. Ugh! She corrected it - no harm done.

 


11/17/04 ~ Well, after receiving my approval a little over a month ago, I had to wait for Barix to get all my paperwork through benefits verification. I now have a date of Tuesday, December 28th. I am feeling so apprehensive right now, like I could have a panic attack. I know it's normal to feel this way, but I am still sooooooo scared. So far, I've read/heard nothing but positive outcomes from Dr. Kaczmarkski's patients at Barix; however, I feel especially nervous because I am asplenic (no spleen). Please PRAY for me that I have no complications or infections, as they could EASILY be life-threatening for me. I just need some positive thoughts right now...

 


12/4/04 ~ I have my pre-admission testing this Tuesday, 12/7 at Barix. I was told I will have an EKG, chest x-ray, and gallbladder ultrasound. I wonder if there will be other tests too?

 


12/8/04 ~ Pre-admission testing was uneventful. Everything went very well!

 


12/15/04 ~ Went to a support group meeting last night at Barix in Langhorne. It was VERY informative, and I met a lot of great people. I left there feeling extremely pumped up and ready for my surgery!!

 


12/23/04 ~ Only 5 days away from surgery, and I am so ready! I did my shopping and stocked up on sugar-free Jello, Wyler's Light sugar-free drinks, Flintstones Complete vitamins, and other stuff. I even went to GNC and purchased a 3 lb. tub of Nature's Best Isopure Low Carb Dutch Chocolate whey protein. Have no idea what this stuff will taste like, but the owner of my local GNC highly recommended it as it does not leave you gassy as some of the other whey proteins do. We shall see...

 


12/27/04 ~ Received a call from Pat at Barix advising that my surgery would be at 12:30 tomorrow. She also said that I might be bumped up earlier and may receive a call in the morning. A few hours later I received another call from Pat letting me know my surgery had been bumped up to 9:30 and that I should arrive at Barix by 8:00.

 


12/28/04 ~ Arrived at Barix around 7:45 a.m. Was feeling very calm and ready to move forward. Had to give a urine sample, then sit and wait. Was then called back one final time for insurance verification, then back out to the lobby to sit and wait. Was then called into a room by the Pre-Op nurse (and an assistant, who I think was training) who made sure I had my living will papers, asked if I'd had anything to eat or drink, and did I have any questions. I was then told to go back to the Lobby and they would be ready for me in about 15 minutes. Waited in the lobby with my husband, still feeling pretty calm. About 15-20 minutes later, the Pre-Op nurses came out for me and I kissed my hubby goodbye. I was brought into the Pre-Op bathroom and given a gown to change into. I then went to lie on a stretcher to have my IV started. This is when I started to feel nervous. The heart monitors were started, and my heart rate was fast and my blood pressure up. I was nervous about the IV stick! Once that was done, I was given a little cup of medicine to drink (I forgot what it was called) which reduces the stomach acids and helps settle your stomach. It tasted horrible but went down quickly. Next, the Ted hose and compression cuffs went on my legs. The anesthesiologist came to see me about 1/2 hour later, talked to me for a bit, and had me sign a consent. Finally, Dr. Kaczmarski's surgeon's assistant came out and talked to me, and then Dr. Kaczmarski came out shortly afterwards and had me sign his consent form. I was in Pre-Op until 10:30 a.m. when Dr. K and his assistant finally came out and said "Ok, we're ready." Next thing I know I was being wheeled out of the Pre-Op room. All I remember is seeing the doors open and THAT WAS IT! I was out like a light! I'm not sure how or why, because I didn't see anything going into my IV line but I assume it was whatever they had given me earlier to relax me. I'm so glad I was out before I even saw the Operating Room! Next thing I remember is hearing Dr. K's assistant calling my name and telling me that surgery was over and I had done great. I was fading in and out, and don't remember anything else until I was already in my room. For pretty much the rest of that day I slept. I had Demerol in my PCA, and I simply couldn't keep my eyes open. Sometime that evening the nurses came in and said it was time to get up and see if I could walk. I remember them helping me out of bed, and as soon as stood up I felt dizzy and nauseous. They quickly gave me the little barf pan, and sure enough I threw up a little bit of blood (which scared me). The nurses assured me this was normal, however. I guess they must've gotten me out the door because the next thing I remember is standing out on the floor outside my room (about 20 feet from my door) and dry heaving. Ugh - it was horrible. I don't remember anything after that. I guess I walked some, or they got me back into my bed. I have no idea! The next day I slept a lot, but did get up and walk and of course do my breathing treatments with the spirometer. I wasn't really in a lot of "pain" - I think it was more "discomfort". On the third day I was taken downstairs around 7:15 a.m. for my leak test. I had to drink this horrific, absolutely disgusting stuff in a little cup and then Josh took about 4 x-rays. That took about 15 minutes, and then he took me back up to my room. More walking, breathing, coughing, sleeping. My x-rays came back fine - no leaks. I started on clear liquids. First water, then Jello, then broth, and juice. I did fine with all except the broth, which went right through me (diarrhea). On Thursday night the nurse gave me a Dulcolax suppository (UGH) and within 10 minutes I had my first bowel movement. It was all very dark green/black, which kinda scared me at first. The nurse assured me this was completely normal. I got up once during the night to go to the bathroom and discovered that I was bleeding out my drain hole. I became panicked and called the nurse. They called Dr. K. who advised to remove the dressings, put ice on the area for 20 minutes, then redress. The bleeding had stopped. I got up again around 4:00 a.m. to go to the bathroom and this time when I went to wipe, I started to bleed again from the drain hole. Called the nurse who fixed me all up again. I was really panicked by this time. All I kept thinking was, I"m going to go home and be afraid to move for fear that I will open my incision. The nurses were great, and they helped ease all my fears. On Friday morning, the nurse came in early (maybe 7:00 a.m.) with my discharge papers. She went over everything with me and said that Dr. Brader would be up to see all the patients shortly (Dr. K. was off this day and Dr. Brader was filling in). I got a shower, got dressed, and waited for Dr. Brader. One of the nutritionists came by to review things with me. Dr. Brader finally came around 10:15 or so. The nurse removed my drain (I was so scared of the pain I would feel, but it was over very quickly and was more of a "quick" discomfort anyway). That was it - I was out of there by 11:00!


2005

1/3/05 ~ I am almost one week post op and feeling pretty good. I only took one Darvocet the day I got home and no additional since. The liquid stage is hard. I'm doing good with water, diluted apple juice, and Jello but broth still goes right through me. I weighed myself this morning and I am down 8 lbs. Of course, this will probably fluctuate so I'm not going to get too wrapped up in what the scale says.

 


1/12/05 ~ Had my two-week follow-up at Barix today. I'm down 14 pounds! Dr. K was pleased. Everything looks great with my incision. Nutritionally, I'm having no problems with the protein shakes and I'm getting in my 64 oz. of fluids each day. Next follow-up with Dr. K is at six weeks (2/7).

 



2/17/05 ~ I have not updated in a while! It is now 7 weeks since my surgery, and I am down 38 pounds! I have gone from a size 22 jeans to a size 16 (and even some 14's). This is AMAZING! I feel great!

 


3/25/05 ~ I am so bad about updating my profile! Well, I'm down to 195, which is a loss of 50 pounds! Only 45 more to go until I reach my personal goal! I am wearing size 14 jeans and mostly Large/XL tops! This feels great!!

 


03/26/05 ~ I'm down to 193! That's a loss of 52 pounds! I feel great!!

 


04/01/05 ~ I joined Curves on May 4, 2004. I lost almost 20 lbs. before my surgery on December 28, 2004. My starting weight was 257 pounds. Today I am 192 pounds. That's 65 pounds lost! Check out the inches I've lost: Bust: 8½" Waist: 10" Abs: 14½" Hips: 8" Thighs: 4" Arms: 2" That's a total of 47" lost! Wholy mackerel !! Can I just say that I FEEL GREAT !!
04/10/05 ~ We put our house on the market and it sold in just three weeks (we knew it would go quickly). We have finally made it through attorney review on our new one (closing June 30th). Over the past several days we have been doing major clean-out of items we no longer need (i.e., at least 100 trips up and down the basement stairs). I AM DOWN TO 186 !!! I cannot even believe it! It's like I lost 10 pounds overnight! My size 14 jeans and shorts are FALLING OFF OF ME! At the beginning of this journey, I figured a size 14 would be about the lowest I would probably go. Now I am really starting to see that I will probably surpass my own personal goal of 150 and shoot for my surgeon's goal of 137. I cannot even believe how "small" I feel. I have NEVER felt this way!! My BMI went from 41 to 29.1 in less than four months! I am down 71 pounds, and I couldn't be happier!!


4/23/05 ~ I'm down to 183! That's a loss of 74 pounds since December 28th (four months). I'm wearing size 13/14 jeans and Large/X-Large tops (depending on the style and brand). BEFORE........................................................AFTER

04/30/05 ~ Down to 180! WOO HOO!!



05/01/05 ~ I just bought the dress below in a size 12 and cannot wait to wear it this summer on our vacation in Ocean City, MD!


05/20/05 ~ Down to 178. The weight loss has really slowed down, but I'm not panicking (yet). I bought a lot of new clothes over the past month, including some cute halter/babydoll tops and a few capri sets, and all of them were too big and had to be sent back (the dangers of ordering over the internet). Then I went and bought some capri pants in the store in size 14, and dang if they weren't falling down! I exchanged them all for size 12 and they fit perfectly! Who would've thunk!


05/23/05 ~ Down to 175! Just when I thought the weight loss was slowing down!! I've been trying to get in more "food" rather than simply protein shakes, and while I'm not sure if this is what has worked or not, I'm gonna keep on doing what I'm doing!


05/25/05 ~ Down to 172 this morning, for a total loss of 85 pounds.





06/19/05 ~Down to 167 and in size 10!


06/27/05 ~ Still hangin' out at 167, which is okay with me. Things have been so hectic! We are closing on our new home in just three days!! I'm so excited! We're moving to Hunterdon County in NJ, which is where I was born and raised! Below are photos of our current home (which we bought in 1999) and our new home!


07/02/05 ~ Well, we did it! We're in our new home! Things went smoothly! The best part is I lost 5 pounds in the past 3 days!! I'm down to 162. I am AMAZED every day at how this tool is helping me.


07/14/05 ~ I'm at 161, for a total loss of 96 pounds. Only 4 pounds away from the Century Club! My weight loss has slowed down, but I must admit I have NOT been working at it AT ALL. Things have been so hectic from our move, and I really need to get back on track. I am running out of clothes that fit me properly - everything is too big. I bought a lot of summer tops and shorts in 14s and 12s, and they're all too big and baggy. Never in a zillion years would I have imagined I'd be buying size 10s! I wanted to start putting together a fall/winter wardrobe, as I have absolutely NOTHING in my closet but I definitely think I'll wait a bit longer because I just don't know what size to even buy.


07/19/05 ~ Well, where do I start? I just spent 5 days in the hospital. I am still in some pain and can't really sit long in this chair, so this will be a very condensed version. I am asplenic (no spleen due to ITP - low blood platelets/autoimmune disorder). Wednesday night I came down with EXTREME chills and fever, called off sick Thursday, slept half the day, had to force myself to get up, by 5:00 p.m. I had the worst back pains of my entire life. At 3:00 a.m. I was in so much agony I could not stand, sit, or lie down. Went to the ER via ambulance. Turns out I had STREP PNEUMONIA, which is a bacterial infection in the bloodstream. Your spleen produces the antibodies which fight off these infections. Well, since I have no spleen, I could not fight it. Thank G-d I got to the hospital when I did or it would have resulted in sepsis when it started attacking my organs. So, 3 CT scans, an MRI, chest x-ray, echocardiogram, etc. etc. later here I am. Worst of all, I am allergic to Dilaudid and can only take morphine. It did NOTHING for me. So I was NEVER pain-free. The back pain is still there, just not as severe. It has been chalked up to a back injury. I am thankful to be alive. My health has gone downhill since my splenectomy in January 2003. Okay, so I weighed myself just now and I'm at 156. Uh, OMG, hello, as I'm typing this I realize I JUST MADE THE CENTURY CLUB! OMG! 101 lbs. gone. I cannot believe it. Only 19 lbs. until my surgeon's goal! WHOLY CRAP.
