Monday, September 26th @ 12:30pm is my consultation with my nurse at the hospital. I'd like to make a list of things I want to have together in order not to slow down the process.For instance:
* call insurance, find out if they require specific forms for the psych evaluation
* schedule bariatric psych evaluation
* call old doctor to get sleep apnea study results from 2008
* schedule appointment with pcp to get bariatric referral
