BCBS of Tennessee PPO approval for sleeve

Has anyone received approval for the sleeve with this insurance? It states the sleeve is investigational, I hope someone can help. If it was appealed how long did it take to be approved? Thanks for any infi I can get.

I have BCBS PPO, not sure if the state matters.  My doctor's nurse said that its the easiest insurance to get approval from.  Just make sure that you comply with the process, and that all of the forms are in order before you send it.  Your doctor's office should know what has to happen.  I was approved within a week.
