But I'm an outspoken, opinionated ***** so I just tell them that they are the ones that changes, not me. I remind them of all the times I had their back in tough situations and tell them they have a choice - they can be my friend or not. I'll sleep just fine at night either way.
Changed my left butt cheek. Tell them "Hell yes I've changed! Speed up or move the hell over because I and my soon-to-be size 6 jeans (You're a guy so insert your goal weight size) are coming through!"

In my case not really. I didn't tell anyone about my surgery except those who needed to know (i.e. my PCP, boss, and two friends who drove me to doctor visits) so maybe they haven't noticed a change because they aren't looking or expecting one.
The only thing people noticed was when I developed an ulcer and I was looking pale. But besides that, not really and I kind of prefer it that way.
No drama from anyone.
I can say I do act different, I'm more outspoken and I nolonger seek an approval, you get what you get.
