Just FYI since you have been waiting forever...typically your insurance company can give you the approval certification number and you can call your surgeon's office, provide that to them and schedule over the phone! There is always a delay between getting the approval from the insurance company to the surgeon's office it seems and you have been quite patient! Just a thought
