I am 4 years postop..and have NEVER taken a PPI..never had a need..still don't

You do NOT have to take a PPI...and honestly, most docs don't want us taking them long-term.  Alas, some of us may have to.  PPIs long-term can have some negative effects in that your stomach doesn't produce enough acid to break down the protein and it could negatively impact your albumin and prealbumin levels.  

So, count yourself lucky if you don't need it!

My doc had us take pepcid for 3 months.  I'm not on anything anymore and have heartburn less often than I did before the sleeve.

2 things - when you stop taking it, if you find yourself getting heartburn/reflux/gerd, get back on the ppi and then wean yourself off.  It helps your stomach regulate acid by itself without causing any problems.  And, if you find you have unexplained hunger after you stop taking the ppi, it could be excess acid.  Take a tums or pepcid or a dose of your ppi and see if your "hunger" goes away.
