Just found out yesterday.  I'm waiting for the letter from the insurance company to arrive and then will schedule the surgery!!!!!!!!!!b

Now I am being told that there is no documented behavioral modification.  Noone is able to tell me what constitues this documentation, so that I can provide it.  Roseann from Dr. Nagle's office will follow up with the insurance to (hopefully) get some answers so we can plan next steps.

I was denied because of lack of documented 5 year medical history.  I was able to find records from 2001-2002 and we submitted those.

I called today and the insurance said it's not finalized yet, but the notes in my file say that my claim will be denied again for lack of a documented 6 months under a physician's care for weight loss.  

I have been going to Northwestern since February (9 months), which is documented!  This is very frustrating.

The paperwork was submitted to the insurance by the surgeon's office.  Roseann is great to work with and very helpful.  I should hear something in the next 2 weeks.

I just found out that my insurance lowered the physiscian supervised criteria to 6 months from 12.  I am now eligible for the surgey and the doctor's office is submitting the paperwork for me.
