I realized the other day that I hadn't been here in awhile and talking with a friend about the types of people you see on boards early on that scare you into thinking Orange Juice will ruin your life post-DS surgery I thought "I need to go back and tell people what it's really ilke to live with the DS."

I'm 14 years out of surgery. I had it in April of 2009. I was 453lbs at my highest weight. I have not been so much as over 165lbs since that first year of weight loss. I also had a 150 CC and my doctor was Srikanth at the Fife Bariatric Center of Excellence here in Washington.

In the beginning I was strict - but also kinda terrified - in my diet. Egg and cheese crust pizzas, eating only the meat off burgers, it was an okay life especially to lose weight quickly. But now I don't much care. I stick to the rule of "Protien first and less sugar and volume eating in my day than I used to."

The things I eat would never tip someone off that I had a weight loss surgery - a snickers bar, a doughnut, white rice, a glass of juice, whole milk, creamer with sugar, mashed potatoes, an elephant ear at the fair. But it's far far less. The DS taught me to listen to my hunger and satiety cues so I don't over eat. I might eat HALF a donut or one donut. I'll have a cup of rice as opposed to four. I'll eat half a pint of Ben and Jerry's not half the quart. Now I would laugh in your face if you tried to get me to eat an egg and cheese only crust pizza. No thank you.

The best part of surgery is that all of my four kids - One born post weight loss - weigh less at their respective ages and so far throughout their lives than I did at those ages. I grew up in a household where everyone got their own large pizza with their own toppings. Now I buy one for a family of five. Everyone gets two slices. If your hungry later have a PB and J. Again, learning to listen to their actual hunger cues has been an invaluable gift and the real working tool part of my surgery.

I know my DS still "works" I get vagus nerve irritation and get sneezy full when I eat something too quickly and fill up too fast.

But basically I just live my life, I never get on a scale, I take vitamins, and get my levels checked. I don't drink any extra protein.

I got the Duodenal switch to have a "normal" relationship with food - because I fu*king love food. And I still do. I love pasta and good bread and to bake cookies and have sundaes for dinner with my kids. And that's what I get to do and I couldn't be happier.

I get about 150 in on average. But I also drink 150-190 grams of protein in powder, pre made shakes, and shots a day. And I eat like eating is going out of fashion. I eat every two to four hours. I can go through a costco sized thing of babybel cheeses in about four days. 

Last time I checked I was getting upwards of 4000 calories and 400 grams of protein and 150-200 grams of carbs a day. 

I eat a lot like Val, I eat protein first and then whatever else of the rest of it I can shove in my face. I long ago stopped eating no carb pizzas and other things, I just generally hate them. I eat double toppings on single bread. 

I'm four years out on the 20th and I have a 150 CC. I have been and remain in a 155-165 window with very little effort.

Some of you here have already become accquainted but since I have a moment of his sleeping I thought I'd introduce him over here.

His name is Owen. But he seems to respond equally well to turtleface and peanut pie. 
He is my fourth kiddo, and the only boy. He sleeps like a champ but only because being smothered in sister kisses keeps him awake all day long. He was born on September 1st  2011 at 3 in the morning a day earlier than his due date and arrived in just four hours.

Just some info for women wondering about pregnancy post-DS:
I was two years out from surgery, I saw my midwife (who delivered the last two of my girls) who was very familiar with RNY patients and had even said she was hearing more and more about the DS. It helps that she works for the same medical group at a sister hospital as my surgeon.

I got quite a few more ultrasounds to make sure he was growing well and proportionately. Got labs done and iron drawn separately twice just to be on the safe side. Aside from feeling like I needed to eat the house down and doing so (I plowed through breads, crackers, cookies, ice cream, and cakes, and probably six or seven 30g protein shakes a day) I had no issues. I put on only 12lbs total but he was well within a healthy weight, especially for the size of babies I generally have; my heaviest baby even when I was heavier was only 7lbs 12oz so 6lbs 9oz was perfectly in line with that. 

I am back in my pre-baby jeans, and well they may actually be a size too big. I'm breastfeeding, and he's growing great, he had gained his hospital discharge weight by his five day appointment. It's a little harder to remember to get those 600 to 1000 extra calories (recommended extra calories for a BFing mama doubled) a day without such present cravings and a couple times now I've felt light headed, headachy and grumpy and cured those feelings with a sandwich. 
 
All in all it was a fantastic, perfect experience, with not a single issue.

 I don't miss anything. I eat everything and anything. I have very few intolerances and it seems my recently completed pregnancy cured me of my lactose issues (for now); hooray for ice cream. 

I certainly eat far fewer of whatever junk food I inulge in. I mean, I used to eat four or hell lets be honest, six, oreos at a time. Now I'll have one. I used to be able to eat a whole large pizza hut pizza and some cheese bread now I can get down two pieces. I used to have a HUGE bowl of tortellini or mac and cheese now I have a cup's worth. 

This is how I wanted to live though. I wanted to be able to enjoy food and enjoy it socially with my family (elephant ears and scones and fried twinkies at the fair for instance).

 I already had seen this specific group of midwives (there are five in the practice), and this midwife specifically, for my second and third child (this is the fourth and final!) when I was incredibly heavy, I gave birth to my 2 year old at 453lbs with no complications, interventions, or medications just fine.

One of the midwives in this practice has had RNY, and they see quite a few bariatric patients in general due to working within the same medical group as my surgeon (and Dr. Oh and I believe one other doc whose name I am blanking on). And while, yes we are different us DSers and RNYers we are sort of fundementally the same in our pregnancy care and the extra management of our labs/nutrition. Though the ultrasound tech that works with my midwives (I have had a few extra ultrasounds to make sure my child is growing proportionately and properly, and he is!) knew of the DS and said "We've seen quite a few women with that surgery now it seems!"

I guess since I am proactive in my health I didn't consider myself high risk (I didn't automatically consider myself high risk at 450 lbs either actually and I had three healthy perfect pregnancies while SMO so I guess I was right). If your current OB or the OB you want to see, or midwife you choose to see is not comfortable or confident in managing or at least working with you to manage your nutrition needs then you may need to go with someone who is a high risk specialist to get the attention to detail you need.

 Your surgeon is my surgeon. I am two years out (as of April) and I am pregnant (30 weeks). I have seen him once during this pregnancy, earlier on in my pregnancy for my yearly with him, everything was fine. When I got my glucose tolerance pulled I also got second labs for late pregnancy drawn. Everything was fine. 

That said, I take what might be called WAY THE HELL over what he recommends and have been doing so from the start. I added Folic acid and four prenatal vitamins to my pill box as well and I take all of this daily, without fail (okay who am i kidding once in awhile I miss that last sacking out before bed dose, I'm not a saint).

Ultimately you don't need to see him. You just need your labs done and an OB/midwife/GP can request those for you, call up his office and tell them you need the lab order. You don't even need to specify really why you need it they will fax it to you immediately. 

I don't even have a high risk OB.  I deal with a midwife who works for the women's health services branch of Franciscan Health (the same hospital/medical group that Srikanth works with). It's a matter of staying on top of things and getting your ducks in a row.

While I am suspicious that your labs are going to remain fine and dandy taking little to no vitamins while NOT pregnant I can tell you that pregnancy is going to suck every single thing out of you. I didn't even have surgery with my last child and wound up anemic, B12 deficient, and partially from the merit of living in the great gray northwest, "criminally low" on Vitamin D. 

If you are keeping this baby you need to get your butt in gear, one way or another.

 I live on a small island, 1000 permanent residents, with one tiny island general store that charges out the wazoo for everything so unless it is really necessary (or you're just getting a drumstic**** cream on a hot day) you don't shop there. 

So, I freeze a lot, buy in bulk, and always buy a ton of whatever it is we use. When I shop I buy four or five things of sausage, and 12 or 13 things of bacon (and  I have quite a freezer stock of bacon >.>). I always get asked ridiculous questions at the check out. "Are you having a party?" "Stocking up for the apocalypse?"

Nope, just live on an island, don't leave frequently because each trip costs me 20 dollars to get home!

You should see the weird looks when I restock our liquor >.>

I got one, they told me not to take any pills larger than my pinkie nail but then gave me this antibiotic for h.pylori that had in RED on the box that the pills were not to be cut or split and they were larger than the dang multis and calcium.

I called, they said, absolutely do not cut the antibiotics in half or crush them EVER so I just shrugged, figured what the hell and started downing pills whole at three days out and never looked back.

I did crush one multivitamin, and if you were wondering, they are made of unadulterated evil the kind that makes you wish you didn't own a tongue.

1. Milk and ice cream if I don't have lactaid around (sometimes even then I still indulge in ice cream). 

2. LOTS of regular iceburg lettuce. I prefer softer greens like baby spinach. But again, I'll eat some lettuce, especially in BLTs. 

3. Drinking sugar. I dunno It seems like a silly rule at this point for someone who eats ALL THE SUGARS and always has, but I drink sugar free drinks only (Soda, tea, coffee, etc.). I just think of it like this: if I am going to starbucks, I don't want a 250g of sugar DRINK when I could be having AN AWESOME BROWNIE instead. 


But...I will and have pretty much eaten everything. Stuff that is tough on people's stomachs (Beef jerky, rice, lettuce and other roughage) as well as things that cause gas (all the flours and sugars)

 On one hand you do say you are going overboard on the carbs. On the other hand...some of us are hungry. 

I am Nic and I am a hungry DSer, I don't consider this a problem. I put away food (especially now that I am pregnant, and still losing through pregnancy hah figure that out) like it is the last food I will eat. And I have been eating since I had hamburger and beef jerky at three weeks post-op. 

The hardest part of my day is trying to figure out if what I am eating is third breakfast or first lunch sometimes. And I eat decent portions, whole BLT sandwiches (with what might be pleasantly described as a fuckton of bacon), a bowl of bacon and walnut topped oatmeal, Three eggs, a quarter of a thing of pork sausage, and a slice of butter drenched high fiber toast are all common single 30 minutes tops sittings of food for me. And I will eat no more than two hours directly after that, earlier if it's really early in the morning. 

So, it is possible to eat and eat and still lose.

 Yes it's normal. Especially if you are keeping yourself well hydrated. A pint of water (16 oz) weighs a pound. 

The large part of your weight "gain" over a day is water, but also you are likely carrying around food mass still in your stomach and intestines.

It would be nearly impossible to gain 7lbs of fat in 12 hours and then lose it all over night.

Heck, I went to the fair and "gained" 11lbs that day...that was all gone by morning.

 The current ones are 7, 5, and 2. They'll be 8, nearly 6 and nearly 3 (they latter two have December Birthdays and newbaby is due early September though my husband thinks this one will come early so we'll have two in August and two in December). 

To tell the total truth I wasn't REALLY on board with a fourth kid AT ALL in the beginning I just felt like I was finally getting to Do all the things that I wanted and live how I liked, etc. But the Hubs was really really adamant to like the point of pouting and begging. And then I decided I definately want to save up for plastics and get that done (and as I'm 29 spend my early 30s look faaaaaabulous! gurl) so I was like OKAY FINE I WILL HAVE THIS KID OKAY AS LONG AS I CAN GET MY STOMACH ALL LIFTED BACK WHERE IT BELONGS YO. And Hubs is getting a Vasectomy in the next couple of months so this will be the lasty last one. 

But now I couldn't be more excited for newbaby, as I said it's kind of like doing Pregnancy for the fourth time (pretty easy, not sick, my body knows what it's doing, but also for the first time (people notice I'm pregnant, treat me differently etc). Pregnancy hormones help with the being excited too, probably >.>

 I do coupons or shop deals when they're out, If there is meat on sale at one of the grocery stores I visit I'll buy it even if we don't NEED it. Mostly this is because I live on an island with one general store that is privately owned that practically scalps for things like eggs and milk so we freeze a lot of things (I'll chop fresh veggies up and freeze them for instance). 

BUT I notice a lot of the extreme couponing stuff, from family that does it, is that a lot of the stuff they buy is pre-packaged sodium filled garbage. I mean sure you can buy flats of condensed soup, for instance, but I try not to use condensed soup I've even started making my own cream of mushroom and freezing it for various recipes. 

I dunno I'm more picky about my groceries than that generally.

AND oddly enough Carb smart tortilla shells or low carb pasta/bread. I can eat all the whole wheat pasta and breads I want but if it says "Low carb" on the package it's some kind of weird voodoo by where my digestive system is like WAIT WAIT GO BACK THAT IS CRAZY!

Although, I rarely have gas that hurts, more I just have to sit at my desk with the air freshener heh.

 It's really mind boggling how quickly everything can normalize. I go about my life day to day as if nothing about me is different, or changed. Things don't strike me as amazing anymore, I don't look toward goals, or think about how I used to not be able to do this! I just buy clothes, eat at restaurants, and have fun with my family like any normal person. I know people who don't know I had surgery, I don't feel compelled to tell them unless they see pictures of me, I'm forthcoming with the information it's just...not important day to day. 

That is until I try to get into a concert with my ID (which doesn't expire for another few years as I renewed it RIGHT BEFORE surgery) and the guy brings me inside, makes me take off my sunglasses, and scrutinizes every detail of the thing. Concedes only after a little bit that "It's you, you can tell around the eyes...but boy is it a different you."

At this point there isn't much new to tell: I lost over 220lbs, I hit goal, I wear a size 12 pants and a size 6 top (It was the same when I was heavier, I wore a size 26/28 top and a size 38 pants).  I'm currently 5 months pregnant - I get to find out if it's our fourth girl or a boy on the 29th, all my labs are spectacular. I eat what I want, it's kind of like a fun hobby, as my nutritional needs are largely addressed by the protein shakes and vites I take. I never have gas or loose stool that I either 1. can't predict or mitigate and 2. is so bad it'd make me stop eating half a lemon poppy seed muffin with my coffee in the morning. I'm pretty reactive to milk, my stomach gets gurly, but I buy darigold 100 percent lactose free and am back on the fruit loops for dessert trolly. I can eat salad, chicken breast, white rice, pasta, sugar, basically (and this is what my surgeon promised me with this surgery) what I WANT like an adult who is allowed to make food choices for herself. 

I am enjoying being pregnant this fourth time around, It's like getting to experience pregnancy fully for the first time. Every previous pregnancy I was 300+ pounds (All of them were spectacularly healthy, there is no reason a heavy gal can't be perfectly fine while pregnant). People comment on the fact that I am pregnant, Older ladies have started to try to touch me, I get to buy maternity clothing: Funny aside here, all my Small and Medium tops were starting to fit tight across my stomach or rolling up so I went to get maternity clothing. Well since Small and Medium in my regular clothes hadn't been fitting correctly for some time I pulled Large in Maternity. Silly me; they fit like tents! I had forgotten my size. So, I went and pulled some Small maternity tops, and LO they fit like magic again!!

I mean all in all this surgery gave me the ability to feel on the outside like I did on the inside. I feel more like myself than I ever have in my life. I feel free to express myself more with fashion; even if it's because there is simply more available in my size, I feel more able to get out and be with my kids. I have more energy, I keep a cleaner house (I think before I was tired and didn't know it, I thought I cleaned, I thought I was pretty on top of things but man, you should see my home now). I'm just ME now, everything is right and people see me for who I am, or at least I do when I look in a mirror.

 Aside from Prenatals I didn't take any vitamins until I was looking to get the DS. They took my base line blood work and found out I was "alarmingly low" in iron, and had low B12 and D. I attribute the Iron (which has come up gloriously post surgery) and the B12 to having just had a baby four months before I had surgery, she took it all the little monster. 

But I started taking vitamins pre-op to get my vitamins trending upwards before I had surgery and it was more difficult to bring them up. I must say I freaking LOVED B12 shots, the nurse warned me that I might, she said that they have patients who beg to have the perscription renewed because they make them feel awesome. They are really like pure energy in a syringe, it's kind of amazing. Lol. 

Since surgery I haven't had any problems taking my pills, I can take two multis two calcium and a handful of whatever else is in that dose for the day in one gulp. I learned the trick here, that you put the water in your mouth first drop the pills in tilt your head forward then back and swallow (the forwad causes the heavy pills to drop with the floaty pills and then back and swallow they all go down whole). I was able to take pills, not chopped up in big old groups like this from day three post surgery and I haven't had any problem since.

 Generally you will hear that Mag Citrate is "for" cramps and twitches and mag oxide is "for" laxative. 

HOWEVER, you get enough of any kind of magnesium it's ALLLLLL "for" laxative. 

Could very well be you don't NEED it. We don't necessarily have the exact same vitamin requirements. I don't take Mag citrate unless I experience night leg cramps.

 How much of the milk did you consume?

I ask because that's where my hang up comes from. So I buy lactose free milk or pop some lactaid. 

Otherwise, from time to time for dessert I'll forgo a brownie or whatever and have a bowl of cocoa crispies, no extraordinary problems for me.

 With the DS, there are no foods that are restricted or prohibited.  We are able to eat what we want. 

I just wanted to highlight this. On my Surgeon's little comparison chart each surgery had a column for post-op diet. And each surgery (band, sleeve, and the two bypass surgeries listed) each had a list of foods to avoid (milk, potatoes, rice, bread, chips, crackers, sugar, etc. basically mushy things and white things). I remember very clearly the DS chart simply said "Quantity and Type of food is least restricted with this surgery." I assume it said quantity since he makes DS sleeves what he calls "considerably larger" than VSG sleeves. 

But yeah, everything else had this long list, whereas the DS had nothing but a statement that food was least restricted. I really keyed into that and liked it.

 My baby was four months old when I had surgery. I bounced back super quick, was out of the hospital in 17 hours from surgery, which is standard for my surgeon, walked around wal-mart on my way home from surgery, and then ate hamburger and beef jerky, the good kind from a butcher, at 3 weeks out. I was terribly low in iron and B12 and had to mega supplement with shots and iron transfusions pre-op thanks littlest kid for taking all that from me!

I didn't really have any problems to speak of, littlest kid though was pretty easy going as a baby.
